City staff can download Major Project Certification Forms on this page.

Pursuant to the Local 21 MOU, please review the list of denied and certified Major Capital Projects prior to requesting certification.

If a project is not listed, submit the “Major Capital Project Certification” form for review by the Capital Planning Program. Forms may be sent electronically to cpp@sfgov.org. Some departments may have additional PDF files of projects over $5 million that have been certified at the Director or Deputy Director level as adhering to the MOU.
